Documentation
There are certain documents that you need for your
Work and Travel programme. All of the documents
are very important and you should photocopy them and put
them in a safe place. If you lose any of the
documents, contact your nearest Council office.
Documents needed to obtain
social security card:

your employer must complete:
I-9: Your employer needs to fill out this form. You should also show
your employer the sponsorship letter that you have been given.

Social Security: You must apply for a Social Security
card. The number on the card is your tax identification number and
you must have to obtain legal work and for your employer to issue
your paycheck. This number is also needed to open a bank account
and/or obtain a state driver's license .
It is advisable that you apply for this card as soon as you
arrive. You can locate your local social security office by
calling the toll-free number at 1-800-772-1213 or at
Social
Security Office Locator and providing them with the zip-code of
where you live or are staying or a place that is easy to find in the
city centre.
Your card should be mailed to you within 10 working days. You can
request a temporary certificate (Form SSA-5028), which is proof that
you have applied for it.

IAP-66 (pink form) is the
'United States Information Agency Certificate of Eligibility', a US
Government document showing that you are participating in the
programme which officially identifies Council as the program
sponsor, describes the purpose of the program and lists the dates
which you are allowed to work. This is a valuable document and you must have this document to
enter the United States.
I-9:
This document is
an Employment
Eligibility Verification, which can be downloaded at
http://www.ins.usdoj.gov/graphics/formsfee/forms/i-9.htm
which verifies that you are legal to work in the United
States.
I-94: Arrival Departure record. A small white card stapled in your passport,
stamped “J-1 D/S” -Sample
I-94 card
J1 visa: This is the visa which is pasted in your passport.
Both your passport and your J1 visas are very important documents.
